Please Note: Children 11 to 16 years of age with listed health conditions will need to book a consultation with their physician to receive written authorization for the vaccine.

Cancellation & No Show Policy
In order to be respectful of the medical needs of the community, please be courteous and call the office if you are unable to attend your appointment.
Appointments are in high demand, and early cancellations will give another person the opportunity to have access to timely medical care. Any cancellations with less than 2 hour’s notice are considered late cancellations that we are unable to fill, and are considered a “No Show”.
We do understand that life does happen, and on occasion you are unable to give sufficient notice for your cancellation. We ask that everyone make a reasonable effort to cancel their appointments when possible. We do offer a cancellation line for after hours if you do not need to reschedule.
If late cancellations or no show’s are re-occurring (3 or more times in a year), there may be a no show fee of $20.00 that we require be paid before seeing a physician.
