We have worked and continue to work collaboratively with various community agencies to pilot provincial community initiatives.

Reduce Impact of Financial Strain (R.I.F.S.)

The R.I.F.S. Initiative is a provincial pilot project in collaboration with McLeod River PCN and our health neighborhood focused on identifying those patients that may have financial concerns, transport challenges, lacking family and community supports, unstable housing situations, loneliness or mental health concerns. For those struggling with income concerns asking for help can be difficult. We encourage a culture where patients are able and feel comfortable discussing what matters to them.

Patients Collaborating with Teams (PaCT)

We work together as a team to identify and cater to what matters to the patient. We know that there your health concerns may not always be completely dealt with by your physician alone. The PaCT initiative allows patients to have a multidisciplinary team work together in supporting you with your care. Whether it be with the dietitian, Home Care, social worker, Mental Health, or your family or support person, we can work with you to identify what matters to you and allow all members of your team to work together to support you in your health journey.

Whitecourt Health Neighborhood

Whitecourt Health Neighborhood is a multidisciplinary wraparound navigation program with the goal of improving all levels of care within Whitecourt and our surrounding communities. We have combined our services to better assist the individuals in need of medical, emotional, psychological or social support in our community.

Life Medical Clinic is working along side other community supports such as Ballad Consulting Group, Canniff and Associates, Healthy Families Healthy Futures, Whitecourt Addiction and Mental Health Services, HIV West Yellowhead, Family Community Liaison Program, Whitecourt Community and Social Services, Whitecourt Family Violence Liaison, Whitecourt interagency food bank, Tennille's Hope Soup Kitchen, Wellsprings, Lorne’s Blanket, and our community Pharmacies to work with those needing additional support within our community.

Help Seeker

Help Seeker is a website designed to support the community with valuable information on how to access support services that may not be otherwise known to those in the community. With the hard work of Mcleod River Primary Care Network, most of the supportive services within our community are listed with information on how to contact the services.
